Certain Controlled Substances & High Risk Medications

Although we do prescribed controlled substances, we do not provide access to high risk medications as well as certain controlled substances. However, we are able to prescribe buprenorphine (Suboxone) for the treatment of opioid-use disorder. Furthermore, we are unable to provide new prescriptions for higher risk conditions like acute cardiac problems, medications for transplants, 'biologicals', hormone therapy (except for birth control), and similar cases. It's important to note that we can only prescribe medication when it is deemed medically appropriate, based off of clinical guidelines commonly accepted by the medical community

Certain Medical Forms

Our medical professionals can furnish you with a work-related note, which may include a return-to-work declaration, as well as Documentation of Recovery. This documentation permits your return to the US following a positive COVID test after recuperation from the illness. Nonetheless, we are not able to complete various other types of legal documents, such as disability forms, workers compensation (DWC) forms, forms granting exemptions from vaccines or mask mandates, and similar forms. Other medical forms can only be provided if the individual is a member of the primary care portion of our practice. Otherwise, those forms must be filled and signed by the primary care provider of that individual.
